How does Rotate PDF turn the file?

Concretely, the page rotation attribute is set in the page dictionary rather than the content being redrawn, so the operation is instant, lossless and reversible, and text stays selectable at the new orientation. Quarter turns and half turns are the common case and take the cheapest path available for the format.
Can I rotate only some pages, or does the whole document turn?

Either. Apply the turn to the whole file, to a page range, or to individual pages — which is the case that actually comes up, when a few landscape pages went through the scanner inside an otherwise portrait document. A page turn is a multiple of 90 degrees by definition here; the format has no arbitrary angle to offer.
Does the format decide whether rotation is lossless?

It does. PDF fixes the page: fonts, vectors, raster images and text coordinates are frozen so every reader sees identical layout. That property is exactly what determines whether a quarter turn is free or costs a re-encode.
Anything specific to PDF I should know?

Yes — a PDF is an object graph, not a page image, so text stays selectable and vectors stay sharp no matter what happens to the raster content inside it. It is the kind of thing that explains why a file looks right in one application and wrong in another.

Is there a file size limit on Rotate PDF?
